Eligibility Requirements
A member is eligible for housing stabilization services if they meet all of the following needs-based criteria:
Be 18 years old or older
Proof of assessed need of service
Be on Medical Assistance (MA)
Be experiencing housing instability
Have a documented disability or disabling condition
At risk of homelessness or institutionalization
